Position: Caregiver Consultation
Hours: 15 hours a week (includes monthly Second Saturday) hourly non-benefit eligible position
Reports to: Becky Zinn-Caulfield, Assistant Director

About the Position: The purpose of the position is to support informal individual and family caregivers through counseling and consultation, accessing internal and external resources and supportive services, among other duties.

The qualified candidate will be responsible for carrying out the following activities:

Individual & Family Resource Consultation (Care Consultation) – Provide direct informal caregiver telephonic, electronic, and in-person assessment, goal setting, plan of action development, resource guidance, education, referrals, coaching, and family meetings, as needed. For caregivers served under Title III-E service definitions, certifications, documentation requirements and protocols required are met.

Caregiver Support Group– Facilitate or support identified caregiver support group. Collaboration occurs with Center leadership and caregivers re: content development, facilitation, evaluation and quality improvement activities, and accurate communications (collaborate with Engagement Coordinator and volunteers, as needed).

Individual and Group Respite– Consultant works in conjunction with Engagement Coordinator re: provision of home-based respite for caregivers. Providing client referrals and ensuring joint home-visits occur and appropriate intake and support provided so that clients and volunteers report satisfaction, Title IIIE measures and protocols are met.

• Other duties as they may be identified or arise upon direction of Assistant Director and Executive Director.


Minimum requirements for the position:

Active social work degree/licensure minimum of BSW and 5+ years’ experience. MSW preferred. Ideally experience and focus on geriatrics, aging or intergenerational family dynamics.

Minimum of 5 years’ professional experience and commitment to providing supportive services to seniors and family caregivers including addressing psychological, emotional and social challenges of aging and caregiving and serving as the bridge of communication between clients, care team, internal and community resources.

Successful completion and maintenance of Caregiver Consultation Certification (Center will support certification).

Knowledge and experience of healthcare delivery, supportive service and healthy aging resource providers/systems in the local nonprofit sector.

Excellent interpersonal, verbal and written communication, boundary/privacy and emotional intelligence skills.

Experience and accuracy of electronic reporting/documentation practice (including timeliness and accuracy of intake, assessments, client encounter notes and other documentation, as required).
