We are looking for a Memory Care Manager whom would be accountable for implementing the Dimensions Memory Care Program, encouraging participation and ownership from all staff at our NEW Assisted Living /Memory Care Building in Hermantown, MN. Ebenezer’s exclusive Dimensions® program stresses the importance of understanding the needs of a person with dementia, and trains caregivers to respond to them with compassion and creativity. This program is designed to ensure that our memory care residents receive exceptional, compassionate care that meets them where they are.
This program coordinator position schedule includes;
- Full time, 80 hours per two weeks
- Day shift
- Monday - Friday; weekends as needed

Responsibilities/Job Description
Using evidence-based practices including:
- Wellness: person-centered, relationship-based care with an emphasis on hydration and nutrition for all dementia patients to ensure their comfort and well being
- Training: delivering innovative, ongoing hands-on education to our staff
- Engagement: Fostering purposeful, pleasurable, creative interactions/activities
- Environment: Creating calm, seeking psychosocial solutions/support
- Education and Support: Providing education and support to families and care partners
- Provides leadership and supervision to staff, volunteers, and/or students.
- Establishes and supports an environment conducive to the provision of quality services and high-level customer satisfaction.
- Implement and maintain the Dimensions Program.
- Develop program and activities reflective of Dimensions Community/Unit resident interests.
- In conjunction with the Executive Director (ED) and DHS/DON, develops budget, routinely monitors financial activities, and maintains accurate financial records.
- Supports a marketing plan aimed at full occupancy.
- Performs other duties as assigned and directed.
- All clinic practice and delegated duties will be directly supervised by the Director of Health Services.
Qualifications
Required
- Associate's Degree Required; Bachelor's degree preferred
- 1 Year experience
- Experience with Microsoft Office
- High level communication skills and ability to interact effectively with older adults
- Active Driver's License and ability to pass MN DOT physical
Preferred
- 1-3 years experience with memory care individuals
