Job Description
Farmington Health Services (aka Trinity Care Center) is seeking a full-time, positive social worker to join our 5-star-rated team. We are a non-profit, skilled nursing facility that is growing to 90 beds within the next year. What sets Farmington Health Services apart isn’t just one thing; it’s everything. Social Worker Responsibilities:
- Admission of residents and the coordination of discharge planning
- Conducting MDS assessments
- Developing and implementing social care planning
- Providing resources, and referrals to residents/families/staff members
Social Worker Requirements:
- Self-motivated to serve residents
- Strong organizational & communication skills
- Able to multi-task well
- Experience with LTC and TCU preferred
Qualifications: BSW / current MN LSW required, two years’ experience in a health care facility preferred.
Hours: Monday-Friday, flexible hours
