The MDS Coordinator is responsible for the
coordination of RAI process and reimbursement process for long term
care.
- This is an ON CALL/CASUAL
position. Requires working 3 shifts per month and 1 holiday per
year.

Lake City Care Center is owned by the Mayo Clinic Health
System with the daily operations managed by Ebenezer. At any given time, as
many as 90 people call our beautiful and vibrant Long Term and Transitional
Care Center their home. We are connected to a Mayo clinic and hospital,
so residents don't have to leave the building for a clinic appointment or

Responsibilities/Job Description
- Lead, supervise, direct and delegate
all RAI functions to ensure staff compliance with federal regulations
- Coordinating the Medicare and
Insurance requirements and MDS completion for the Transitional Care unit.
- Work with the IDT team on
discharge schedules
- Medicare denials, insurance communication,
communication with admissions and the billing office, and coordination and
management of the RAI process (both OBRA and PPS) for TCU
Qualifications
- Three to five years previous MDS
experience in a long term care setting
- Minimum Associates Degree, BSN
preferred
- Current MN RN license
- Current CPR certification
- ANACC Certification preferred

Vaccination Update
As part of our ongoing efforts to protect the wellbeing of our teams and to
provide a safe and healthy environment for the residents and communities we
serve, all Ebenezer employees are required to receive the COVID-19 vaccination
series. [NOTE: the COVID-19 vaccination requirement does not apply
to Ebenezer Homeowners Association (HOA) employees and Ebenezer Florida
employees. Annual Flu vaccinations, while not required, are highly recommended
for all Ebenezer staff..
Ebenezer new hires who are required to be vaccinated for COVID-19 and are
currently not fully vaccinated against COVID-19 are required to receive their
first dose prior to their start date and their second dose (if receiving
Moderna or Pfizer vaccines) within 4 weeks of their start date. Accommodations
