    Overview

    The MDS Coordinator is responsible for the coordination of RAI process and reimbursement process for long term care.  

    • This is an ON CALL/CASUAL position.  Requires working 3 shifts per month and 1 holiday per year.

    Lake City Care Center is owned by the Mayo Clinic Health System with the daily operations managed by Ebenezer.     Responsibilities/Job Description

    • Lead, supervise, direct and delegate all RAI functions to ensure staff compliance with federal regulations
    • Coordinating the Medicare and Insurance requirements and MDS completion for the Transitional Care unit.
    • Work with the IDT team on discharge schedules
    • Medicare denials, insurance communication, communication with admissions and the billing office, and coordination and management of the RAI process (both OBRA and PPS) for TCU

    Qualifications

    • Three to five years previous MDS experience in a long term care setting
    • Minimum Associates Degree, BSN preferred
    • Current MN RN license
    • Current CPR certification
    • ANACC Certification preferred

    Vaccination Update

Annual Flu vaccinations, while not required, are highly recommended for all Ebenezer staff..

    Ebenezer new hires who are required to be vaccinated for COVID-19 and are currently not fully vaccinated against COVID-19 are required to receive their first dose prior to their start date and their second dose (if receiving Moderna or Pfizer vaccines) within 4 weeks of their start date.
