Mount Olivet Home, located in beautiful South Minneapolis, is looking for a long-term care Licensed Social Worker (LSW). The LSW will provide services to our 5-star rated board and care facility with a holistic and person-centered approach. The full time role of the Licensed Social Worker is to create an environment in the facility that provides opportunity for the residents to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners, which will enhance the quality of life for residents and will promote the health of the community. The position is Monday through Friday, with some scheduled flexibility and work-from-home options within each month.

Responsibilities:

  • Know each resident and their family including a psychosocial profile of the resident and an understanding of the family members and dynamics.
  • Completes an in depth assessment of each resident.
  • Participates in care planning for resident needs.
  • Includes families in the planning process.
  • Documents in accordance with the organization’s policies.
  • Initiates upon admission inventory of residents’ belongings.
  • Participates in admission and discharge planning
  • Performs any other duties as assigned.

Qualifications:

  • Minnesota Licensed Social Worker
  • Prior experience preferred, but is not required

Job Type: Full-time

Salary: $24.00 - $28.00 per hour
