Location Information
One of few post-acute care options located within a hospital setting, Transitional Care by Saint Therese occupies 32 private, skilled nursing rooms on the fourth floor of North Memorial Health Hospital. Cross-functional teams provide the very best in care to patients transitioning from hospital to home.

Provides direction and support for client services on the Transitional Care Unit. Evaluates social factors relating to rehabilitation requirements, and provides personal counseling and referral services to residents of the facility. Evaluates and attends to the psycho-social needs of residents. Performs tasks and makes daily decisions to contribute to the day to day functioning of the Social Work role on the Transitional Care Unit (TCU).

  • Attends appropriate Social Services related workshops and seminars. Applies learned knowledge and educates others.
  • Participates in professional organizations related to Social Services.
  • Meets daily with nursing staff to evaluate resident needs and progress.
  • Assures discharge planning begins the day patients are admitted and is designed by patients and their families assuring they have a positive experience once discharged from Saint Therese.

Special Knowledge, Skills and Abilities:

  • Demonstrate working knowledge of Social Services related Federal and State regulations.
  • Demonstrate abilities to participate in and collaborate with groups
  • Ability to positively manage and communicate change and to model appropriate behaviors to staff and others
  • High level of commitment to and success with effective customer service skills
  • Effective time management/prioritization and problem solving skills
  • Excellent written and verbal communication and presentation skills
  • Licensed by the State of Minnesota as an LSW

Bachelors or better in Social Work or related field.



Previous social work experience required.
