The Social Worker is responsible for identifying residents’ needs and preferences, customary routines, concerns, and choices through assessments and care planning.
Identifies and determines the best options that meet the residents’ physical and emotional needs. Identifies, provides services, and conducts interventions for grief, bereavement, and end of life issues. Assists in vulnerable adult maltreatment investigations and provides associate education.
- Bachelor’s degree from a school accredited in the Council on Social Work Education (CSWE).
- Current state licensure the Board of Social Work, in good standing.
- Hospital or nursing home experience.
- Geriatric population experience.
This position is for Transitional Care/Short Term Stay; experience on a TCU is preferred.
